We provide an urgent out of hours service to prevent people going into hospital or an emergency are home placement at a time of health or social care crisis. We also receive referrals from the hospital for patients they want to discharge but who have, or are going need, care for either short term reablement/rehabilitation or longer term support.
All new prospective clients will be involved in an assessment to identify their needs and what support they want us to provide. This assessment will be undertaken by experienced, Senior Management staff. If appropriate, family members or other professionals such as social workers will be asked to contribute to this. After the assessment, Supreme Home Care will propose a plan of care and support and confirm that they have carers who are able to meet the needs of the client. The timing of calls and will be agreed with the client and they will be introduced to their carer/team of carers depending on the complexity of the visit programme.
Clients needs and preferences are kept under regular review and they will have their Support Plans updated as necessary.
